YOUR ADMISSION

By the time you receive this booklet, you have probably already been admitted to our hospital.  During the admission interview, you were asked questions about your personal and medical history.  You may have discussed financial arrangements.  Although some of the questions may appear to be nonessential, they all were important in completing the hospital’s record-keeping requirements.  We appreciate your cooperation.

All patients are admitted, assigned accommodations and treated without regard to gender, race, color, national origin, religion, age, disability or payment classification.

During your stay, you will need a few personal items and we strongly urge that you leave all valuables at home.  In emergency situations patient valuables can be stored in a secured area in the host hospital and returned upon discharge.  Under no circumstances will our hospital be responsible for money or valuables kept in your room.

No electrical appliances may be brought into this facility without prior approval.  The staff can have these items safety checked for you.

During the admission process, you or your designee were asked about and presented information concerning Advance Directives and Organ/Tissue Donor requests.  If you have any questions about Advance Directives, please feel free to contact our Social Worker.

FAMILY REPRESENTATIVE

Each patient family needs to select one person to receive information concerning the patient’s condition and care.   This individual’s name and telephone number will be given to nursing and management staff.  This individual will be called for any and all updates on the patient and only this individual may call and get information concerning the patient each shift  (two times a day).  All other family members and friends will get patient information from this designated individual only.

YOUR VISITORS

Rest is an important part of your treatment.  Your guests are welcome, and we want you to enjoy the pleasure that comes from a reasonable number of brief and friendly visits.  We ask that you and your visitors respect the needs of all of our patients, especially those of another patient who may be sharing your room.  We ask that your visitors please abide by the following:

·        No more than two visitors per patient room during a visit.

·        Only visit between the hours of 1:00 p.m. to 8:00 p.m., seven days a week.  Due to the critical condition of some of our patients, these hours may be altered at the discretion of the charge nurse.

·        All visitors must wash their hands before entering or leaving the patient room.

·        Children under 14 years of age are not routinely allowed in patient rooms.  If you wish that an exception be made, please speak with the Manager of Clinical Services.

·        Our visiting hours are liberal, but we are unable to provide overnight accommodations in the hospital for your family. 

·        There will be specific times that visitors will not be allowed in the patient’s room due to specific procedures being performed at the bedside.

·        If you plan to visit in the evening, or if you ever need access to the building during non-business hours, please notify security in advance.  Entrances and exits are secured after 8 p.m.

·        For any questions, concerns or special requests related to this visitor policy you must contact the Manager of Clinical Services.

·        For out-of-town patient’s visitors accommodations through YOUR PLACE MINISTRIES may be available by contacting Rev. Joe Boone at (859)653-7609 or Cardinal Hill’s social service at (859)371-3880.

Phone service and Television

Local: dial “9” plus the number and these outgoing calls can be placed at any time.  Calls to patient rooms may be received from 7 a.m. to 10 p.m., to ensure an adequate amount of quiet time for patients to rest.

Television and local phone service is automatically provided to our patients for free.  Have the Admissions Coordinator or the Case Manager contact our host hospital to ensure there is no charge incurred.

MEDICAL AND SPECIALTY SERVICES

NURSING SERVICE: 

Your nursing team is staffed with highly trained and skilled individuals who have extensive backgrounds in critical care, rehabilitation, and long-term care settings.  The nursing team consists of Registered nurses (RN), Licensed Practical Nurses (LPN) and Certified Nurse Aides (CNA) and many are credentialed in Advanced Cardiac Life Support (ACLS).

PHYSICAL/OCCUPATIONAL THERAPY:

Patients at this level of care require extensive physical and occupational therapy as part of their healing process.  Licensed and certified Physical and Occupational Therapists as well as Physical Therapy Assistants (PTA) and Certified Occupational Therapy Assistants (COTA) that specialize in acute hospital stays will provide any necessary rehabilitation services. 

RESPIRATORY CARE:

Respiratory care is instrumental in the evaluation, treatment, management and preventive care of patients with cardiopulmonary problems.  All respiratory therapy procedures in this facility are provided by qualified respiratory care practitioners who are here 7 days a week, 24 hours per day and work under the leadership of a pulmonary medicine physician.

CASE MANAGER:

A Case Manager coordinates the ongoing clinical management of the Interdisciplinary Team Plan of Care, including the Discharge Plan for all patients.  The Case Manager communicates with your physician regularly to assist in the coordination of your care.  They make referrals to community agencies, and arrange for services or special equipment for use after your discharge from our hospital.

SPEECH -LANGUAGE PATHOLOGY:

Certified speech - language pathologist are available for patients who require therapy for communication and swallowing conditions.
