The Cardinal Hill Adult Day Care serves persons over 18 years of age who have medical, physical, social, and/or cognitive disabilities. The service area comprises Northern Kentucky and the surrounding counties. Hours of operation are 7:30 a.m. to 5:30 p.m., Monday through Friday, excluding major holidays.
There are three major goals of the Adult Day Health program at Cardinal Hill of Northern Kentucky: To provide daytime nursing care and supervised activity based on rehabilitation principles to medically compromised and/or disabled adults in order to maintain their ability to function in the home and community; to provide respite for families/caregivers in order to reduce the physical, emotional and financial stress experienced during long-term care, and to prevent unnecessary or premature institutionalization of persons appropriate for the intermediate level of nursing home care. The program operates in compliance with the standards recommended by the National Adult Day Services Association (NADSA) and the licensing regulations of the Commonwealth of Kentucky.
Services provided by the program include, but are not limited to:
• Medical and Nursing Services
• Individual plan of care that includes: A health and functional assessment; a plan of care that
identifies the person’s strengths and needs, specific goals to meet the needs, and activities to reach
the goals; notes on progress toward goals; reassessment; and a discharge plan and summary
• Administration of medications
• Supervised rest periods (if needed)
• Emergency medical care plan and provision of first aid
• Consultation with and referral to personal physician as needed
• Monitoring of health status (e.g., vital signs, intake/output monitoring, monitoring blood sugar level)
• Activities to promote health maintenance
• Carrying out or supervising simple treatments ordered by the individual’s personal physician (supplies not provided)
A registered/licensed practical nurse(s) is available to the program staff and individuals at all times.
• Provision of a hot midday meal meeting 1/3 of an adult’s daily nutritional requirement. Meals are
provided by Senior Services of Northern Kentucky. Non-sugar diets are provided if necessary.
• Provision of fluids and nutrient-dense snacks at mid-afternoon.
• Assistance with eating as indicated by the individual’s plan of care. Supplies and devices needed for
special feeding are to be supplied by caregiver.
• Assistance as needed with activities of daily living (mobility, eating, grooming, toileting) and
supervision of personal hygiene.
• Promotion of optimum physical independence within the limits of the individual’s impairment.
• Evaluation and/or consultation by licensed physical, occupational, and/or speech therapists with
physician authorization.
• Daily program of guided exercises.
• Reality orientation.
• A balanced activity program with opportunity for individual choice of activities based on abilities and
preference.
• Group and individual activities such as musical programs, films, games, crafts, speakers and visitors.
• Community Outings
• Health education
Daily or hourly rates can be arranged. Other payment sources may include:
• Medicaid
• Home and Community Based Waiver
• Supports for Community Living Waiver
• County and State funding
