Patient Bill of Rights

As the patient or parent of a patient you have the following rights:

    1. You have the right to considerate and respectful care.
    2. Following an evaluation, the results will be discussed with you. A copy of the written report is available upon request.
    3. After the evaluation, you will be involved in the development of treatment goals, and a written plan will be developed.
  1. You have the right to obtain complete, current information regarding progress made in therapy.
  2. All communications and records pertaining to your care will be treated as confidential.
  3. You have the right to expect reasonable continuity of care. You will know in advance if any referrals or appointments are made by our staff.
  4. When goals are met, or progress has slowed, discharge will be considered. You will be informed of the discharge date and plans.
  5. You have the right to examine and receive any explanation of your bill regardless of the source of payment.
  6. You have the right to read your medical record, if it is the property of this facility. To ensure confidentiality, no records will be released without your permission, except to your referring physician/agency and discharge providers. There is a charge for sending medical records to additional individuals or agencies.
  7. You have the right to file a grievance if you feel that the therapy or therapist does not meet your expectations. Please call the facility to obtain a copy of this form.
