Cardinal Hill Rehabilitation Hospital’s state-of-the-art facility and skilled, caring professionals create an atmosphere that supports the orthopedic patient's recovery and progress.

Discharge planning begins upon admission to achieve the goal of helping the patient return to home and independence as quickly as possible.

Family Involvement

Family members are an essential part of the treatment team, and their involvement is vital to the patient's success in rehabilitation.

We encourage the family to visit, observe, participate in educational sessions and ask questions throughout the patient's rehabilitation program.

Family members also work closely with the Cardinal Hill case manager to ensure an uncomplicated transition from our rehabilitation center to the patient's home and community.

Treatment Team

A multidisciplinary approach to care ensures that the patient's progress is as smooth as possible. The rehabilitation team members include:
  • The Patient & Family
  • Physiatrist (Physician specializing in Rehabilitation Medicine)
  • Rehabilitation Nurse
  • Physical Therapist
  • Occupational Therapist
  • Speech/Language Pathologist
  • Case Manager / Social Worker
  • Psychologist
  • Dietician
Internal Medicine consultation, vocational evaluation, recreational activities & pastoral care are also available.

Individualized Treatment Team

Each patient's plan of care is designed to meet his individual needs.

In addition to the training in mobility and self-care skills provided by licensed Physical Therapists and Occupational Therapists, the plan of care may include any or all of the following services:
  • Wound care
  • Home management retraining
  • Evaluation of equipment needs
  • Community reentry activities
  • Diet & medication teaching
  • Adjustment counseling
  • Vocational evaluation
  • Driving evaluation
  • Evaluation of the patient's home environment
  • Community referrals
The team members focus on helping the patient regain the ability to live life as independently as possible, revising the patient's treatment plan as he meets each new goal.

Follow-up Services

At Cardinal Hill, our relationship with the patient does not end at discharge.

Outpatient services can include selected therapies, vocational evaluation and driver's evaluation.

The patient may also be scheduled to return to Cardinal Hill's Outpatient Clinic for physician and nursing follow-up.

Community Partnership

Kentucky AgrAbility
Cardinal Hill is a partner with the University of Kentucky AgrAbility program. AgrAbility is designed to help farmers keep farming after illness and injury. AgrAbility provides services such as: on-farm help like modifying equipment and tools, help finding financial support for assistive technology and farm modifications, and educational programs for farmers.
