Patient Information

Cardinal Hill Rehabilitation Unit is there when you need us. Whether your stay was planned or spur of the moment, our team of specially trained staff will help you get back to an active lifestyle.

Below you will find information about what you can expect during your stay with us.

 

Information for Patients

Admission to Cardinal Hill Rehabilitation Unit

An Admission Facilitator will do an assessment and provide you with information about the Cardinal Hill Rehabilitation Unit before admission. An arrival time will be scheduled for admission day. Plan to have a member of your family accompany you to the Cardinal Hill Rehabilitation Unit  to help you get settled into your room, to tour the unit and to meet staff. (He/she may stay until 8 p.m.) Your family member should bring along a completed Patient and Family Pre-Admission Questionnaire, as well as your personal belongings and insurance cards. Cardinal Hill will arrange for your medical records to be sent to the unit before you arrive.

Food Service

Meals

Meals are served in the patient’s room. Patients receive assistance with eating, if needed.

Meal times for patients are:
Breakfast         7:30 - 9:00 a.m.
Lunch              11:30 - 1:00 p.m.
Dinner             5:00 - 6:00 p.m

Patients who are in special eating retraining groups are scheduled for particular times within these meal periods.

Any food brought in to patients from outside the unit must be stored in airtight containers unless it is consumed immediately. Please check with your Nurse or Dietitian to be sure that food brought into the unit from an outside source conforms to any dietary restrictions you may have.

   

For Your Safety & Security

Medications


Only those medications prescribed by your Cardinal Hill Physician are permitted while you are in the unit. If you are admitted from home, please bring with you all medications you have been taking so that they can be checked, and medications then will be ordered by your Cardinal Hill Physician and supplied from Cardinal Hill's pharmacy.

You and your family will be instructed in the use of medications needed on an ongoing basis, and you will receive prescriptions for these at the time of discharge.

Going Home From the Unit

Discharge and Beyond


Going home after a stay at the Cardinal Hill Rehabilitation Unit is a special event.  We call these celebrations, rather than discharges.  On your celebration day, your Case Manager will help you check out. After you have been discharged, we want to make sure that you continue to do as well as possible. Your Cardinal Hill Physician will send a summary of your treatment and progress to your home physician. If further therapy or help at home is needed, your Cardinal Hill Case Manager will refer you to the Cardinal Hill Center for Outpatient Services, to outpatient facilities in your home area, or to the home health agency of your choice. The team will make sure that your family has detailed instructions for your home program or exercise and care.

You may be scheduled to return to see your physician at the Cardinal Hill Rehabilitation Hospital Outpatient Clinic following discharge. The purpose of the clinic visit is to review your progress at home and help with any problems that may have occurred. In addition, you will need to reestablish contact with your home physician so that he or she can assume primary responsibility for your ongoing medical needs. We are always available to answer questions or help with a problem. Please let us know how you are doing!

Grievance Procedure


It is the staff’s sincere hope that we can work together to meet your rehabilitation needs. Your opinions and ideas are important to us in planning and carrying out your program. If you or your family have a problem or concern about your care or your program, please let us know so we can work together to resolve it.

The first step in the grievance procedure is to discuss any concern with the person(s) involved or your Case Manager. Most problems can be resolved in this manner. However, because we realize this is not always possible, management and administrative staff may be involved in the resolution of grievances, when necessary.
