Cardinal Hill Home Care is a service that provides comprehensive nursing and therapy care in the home setting.

Cardinal Hill Home Care offers the full array of quality home care services with an emphasis on the intensive therapy services for which Cardinal Hill Rehabilitation Hospital is known.

We combine the latest home rehabilitation advances with a warm and caring approach.

Cardinal Hill Home Care opened in January 2004 and is owned and operated by Cardinal Hill Rehabilitation Hospital, a division of Easter Seals Kentucky aka Cardinal Hill Healthcare System.

Becoming a Home Care Client

Cardinal Hill Home Care offers services to patients without regard to race, creed, disability, religion or color.

Services are available to patients in the following Kentucky counties:

  • Fayette
  • Franklin
  • Jessamine
  • Madison
  • Woodford

The patient's physician must prescribe home care services and the patient must be homebound, but does not have to be confined to bed.

In addition, the patient and the family must be willing to work with the Cardinal Hill Home Care staff in order to improve the patient's health.

Family Involvement

Family members are an essential part of the treatment team, and the involvement is vital to the patient's success in rehabilitation.

We encourage the family to observe and participate in educational sessions and ask questions throughout the patient's home care program.

Family members will work closely with the Cardinal Hill Home Care case manager.

Individualized Treatment Plan

A plan of care is designed to meet each patient's individual needs. The team members focus on helping the patient regain the ability to live life as independently as possible, revising the patient's treatment plan as each new goal is met. The plan of care may include any or all of the following services:

Skilled Nursing

A registered nurse sets a plan of treatment with the doctor, patient and family. Nursing services may include physical exams, wound care, catheter care, IV therapy, tracheostomy care, colostomy care and patient and family teaching.

Physical Therapy

A licensed physical therapist will work with the patient to meet the patient's needs, which may include pain relief, prevention of further disability and/or restoring function for maximum independence. Each therapist will customize a home exercise program that meets the patient's specific needs.

Occupational Therapy

A licensed occupational therapist will work with the patient to improve activities of daily living. This may include increasing independence with self-care and other functional tasks.

Speech-Language Pathology

A licensed speech-language pathologist will work with those patients requiring assistance with speaking, pronunciation, comprehension, reading, writing, memory, cognition, and swallowing.

Medical Social Work Services

The social worker will work with the patient and family to address personal, social, emotional or financial needs related to the illness and their progress.

Home Health Aide Services

These services serve as a supplement to those provided by the other home health clinicians. Aides may assist with bathing, grooming, routine exercises and light household tasks.

Payment

Cardinal Hill Home Care staff will work with the patient and family to assess the patient's payment options.

In most situations, Medicare, Medicaid and private insurance (if this is a benefit of the client's policy) will pay for home care services.

Private payment may also be arranged
