What to Bring to Your First Outpatient Therapy Appointment - New Patients

There are several forms that you will need to have filled out at your first appointment.  You can print them off here, fill them out and bring them with you.

Adult Patients
 Outpatient Adult Medical History Form
Be sure to list all of your medications on your Outpatient Adult Medical History Form.  If your doctor's office gave you the order for therapy and did not fax it to Cardinal Hill, please bring that with you.  You will also need to bring a picture ID and your insurance cards.

Pediatric Patients
 Outpatient Pediatric Medical History Form
Be sure to list all of your medications on your Outpatient Pediatric Medical History Form.  If your doctor's office gave you the order for therapy and did not fax it to Cardinal Hill, please bring that with you.  You will also need to bring a picture ID and your insurance cards.

Cardinal Hill is a partner with the University of Kentucky AgrAbility program. AgrAbility is designed to help farmers keep farming after illness and injury.
