What to Bring to Your First Outpatient Therapy Appointment - New Patients

There are several forms that you will need to have filled out at your first appointment. You can print them off here, fill them out and bring them with you.

Adult Patients
Outpatient Adult Medical History Form
Be sure to list all of your medications on your Outpatient Adult Medical History Form. If your doctor's office gave you the order for therapy and did not fax it to Cardinal Hill, please bring that with you. You will also need to bring a picture ID and your insurance cards.

Cardinal Hill Outpatient Services Information

Cardinal Hill Rehabilitation Hospital has an outstanding Outpatient Therapy program that treats patients of all ages in a varitey of settings.

Adult outpatients are treated on the ground floor of the hospital. Several gyms and treatment rooms are available to treat each patient to address his/her specific needs. The pool is also located on the the ground floor.

Cardinal Hill is known for being a leader in providing rehabilitation services specializing in the treatment of spinal cord injury, traumatic brain injury, stroke and other neurological conditions. Pulmonary and orthopedics are other areas Cardinal Hill specializes in. We see patients with upper and lower amputations as well as hip and knee replacements.

Cardinal Hill's therapists provide services for all patients whether they have a mild injury or a severe disability. Not only do we treat patients with neurological diagnoses, but we also treat patients with orthopedic injuries such as sports injuries.
